Careforth Expands Offering of Coordinated Caregiving Services for Family Caregivers in North Carolina

Available in 36 counties, new service provides financial assistance, expert coaching, and emotional support to caregivers of people of all ages.

Careforth, a leading provider of caregiver support and resources, announced today that it is expanding its family caregiving services to include caregivers of people of all ages in North Carolina. Available to those who care for a loved one at home, Careforth’s financial assistance, expert care team, and emotional support services is now open to caregivers of people ages 0-20 under North Carolina’s Community Alternatives Program for Children (CAP/C).

Under the Community Alternatives Program for Disabled Adults (CAP/DA), Careforth already provides support to caregivers in 36 North Carolina counties who care for their loved ones, including adults age 18 to 64 with disabilities, and adults age 65 and older. Now, with this extension in services under CAP/C, Careforth is able to provide its services to the caregivers of medically fragile and medically complex children who are 0 through 20 years of age.

There are individuals across the state, young and old, who depend on caregivers each and every day. According to the North Carolina Medicaid Division of Health Benefits, the following criteria determines eligibility qualifications for CAP/C:

  • Medically fragile and medically complex children age 0 through 20 years of age.
  • Is determined to require a level of institutional care under the North Carolina Medicaid State Plan.
  • In need of at least one or more CAP/C home-and community-based service based on a reasonable indication of need assessment that must be coordinated by a CAP/C case manager.
