Careforth’s Caregiver Homes Network Receives Case Management Accreditation with Long Term Services and Supports Distinction From National Committee for Quality Assurance

The three-year endorsement demonstrates Careforth’s commitment to support caregivers at every turn.

Careforth, an innovative caregiving company that empowers and enables thousands of family caregivers across America to confidently care for their loved ones at home, announces that its Caregiver Homes Network Structured Family Caregiving service has again received a three-year Accreditation for Case Management with Long Term Services and Supports (LTSS) Distinction from the National Committee for Quality Assurance (NCQA). The three-year accreditation is the highest Accreditation status awarded by NCQA and demonstrates strong performance of NCQA’s rigorous requirements for case management.

Earning NCQA’s Case Management Accreditation with LTSS Distinction demonstrates that an organization is dedicated to coordinating the delivery of care in a person-centered and integrated manner to help Individuals function optimally in their preferred setting. NCQA Accreditation standards are purposely set high to encourage organizations to continuously enhance their quality.

“Receiving this endorsement is a testament to the dedicated Care Management professionals at Careforth,” said Careforth President and Chief Executive Officer Matt Marek. “It’s truly an honor for an organization like NCQA to recognize our commitment to delivering high quality case management to the families in our Structured Family Caregiving service. We intend to keep using the NCQA standards as our North Star for continuous improvement.”

NCQA awards the status of Case Management Accreditation with LTSS Distinction to organizations that demonstrate strong performance of the functions outlined in the standards. NCQA is widely respected and associated with high quality health care and has developed Accreditation standards with input from researchers in the field, its Case Management Expert Panel and LTSS Advisory Committees, employers, both purchasers and operators of Case Management and LTSS programs, state and federal regulators and other experts.

“Case Management Accreditation moves us closer to measuring quality across population health management initiatives,” said Margaret E. O’Kane, President, NCQA. “Not only does it add value to existing quality improvement efforts; it also demonstrates an organization’s commitment to the highest degree of improving the quality of their patients’ care.”
