Seniorlink and Humana Pilot Program featured in OPEN MINDS

Seniorlink was recently featured in a news report by OPEN MINDS, a company which provides resources for the health and human services industry, in an article titled, “Humana & Seniorlink Pilot Medicare Advantage Virtual Care Team Program.” The pilot specifically targets those Medicare Advantage members who have multiple chronic conditions, struggle with daily activities, and are frequently hospitalized.   “The goal is to improve care management for up to 250 members in 34 states, and to allow these members to live outside of senior care facilities,” according to the article. The pilot will connect the member’s family caregiver to professional care managers virtually through Vela, Seniorlink’s care collaboration app.
