ReGister your alternate caregiver

You deserve a break.
So you can come back
stronger.

We understand the duties of caregiving can be all consuming, but it is really important for you to take breaks. We’re here to remind you how important it is to have a backup caregiver and a great benefit as a caregiver in Adult Foster Care.

Massachusetts AFC Alternate Caregiver Program provides:

  • 14 Paid Days off for primary caregivers. To receive payment you much have an approved alternate caregiver in place.
  • 14 Paid Days off for backup caregivers. Approved backup caregivers also get paid for each day that they provide care.
  • Benefit resets every January 1st.

Here’s how to get started

1. Choose a Backup

Identify your backup caregiver. Select a friend, family member or neighbor you trust. You can change your alternate caregiver later too. Discuss the role and what you do each day to ensure they are willing to take on that responsibility.

2. Provide Contact Info

Fill out the form above with their contact information. Your care team will follow-up with you first and then reach out to the alternate caregiver to start the registration process. Note that this process can take several weeks.

3. Get in Touch

To speed up the process, have your backup caregiver call your care team directly so that required documentation can start to be shared.

Expectations of the backup, alternative caregiver:

  • Perform the same daily responsibilities as the primary caregiver.
  • Need to download and use the Careforth app to submit the daily note (backup caregivers will be given access to the app with their own username and password and receive training from the Careforth team).
  • Pass a background check and provide a recent physical within the last 12 months at the time of signup, along with some additional requirements.

“Taking care of a loved one can take an emotional and physical toll on the caregiver. Having the ability to take time away and know that your family member will be well taken care of allows you to rest and recharge in order to continue providing quality care.”
