Support we deliver

  • Care Coaches are accessible anytime through our secure app and by phone
  • Care Coaches are fluent in English and Spanish, with many other languages supported through a translation partner
  • Emotional support through active listening, reframing challenges, and empowering caregivers to advocate for themselves
  • Many Care Coaches are RNs -trained in medical conditions and caregiver protocols
  • Over 10,000 informational articles on specific medical conditions with clear instructions and practical advice -all written for you, the caregiver
  • Information on the rights and abilities caregivers have within the healthcare system
  • Help navigating the complex Medicare/Medicaid landscape
  • Care Coaches connect caregivers to many available resources in your community and help you access them -thinking through and overcoming barriers
  • We also connect caregivers to resources for specific health conditions whenever available
  • Your Care Coach will coordinate with providers and family members, often making phone calls on your behalf
