Careforth Earns National NCQA Accreditation for Case Management (LTSS)

When families turn to Careforth for support on their caregiving journey, they deserve to know they are receiving the highest quality of care from a trusted partner. That’s why we’re proud to share that Careforth’s Structured Family Caregiving program has earned a three-year accreditation from the National Committee for Quality Assurance (NCQA) for Case Management for Long-Term Services and Supports (LTSS). This recognition honors our commitment to delivering coordinated, person-centered care for the families we serve across the country. 

A Decade of NCQA Recognition

This milestone builds on more than a decade of NCQA recognition. Careforth first earned the NCQA Case Management Accreditation in 2014, renewed that accreditation and added an LTSS distinction in 2017, and successfully completed renewal surveys in 2020 and 2023. Careforth is one of just 76 organizations nationwide to hold the three-year accreditation for Case Management for LTSS.

What This Accreditation Means for Families

NCQA accreditation is among the highest recognition an organization can receive in case management. It shows that Careforth meets national benchmarks for person-centered care, care coordination, safety, and quality improvement. More importantly, it gives families, healthcare partners, and community organizations added confidence that they’re working with a trusted organization dedicated to delivering exceptional support.

What is NCQA Case Management accreditation for LTSS?

NCQA’s Case Management Accreditation for Long-Term Services and Supports evaluates how an organization delivers LTSS—including care coordination, safety, and quality improvement—against national standards. It confirms the organization provides coordinated, person-centered care that helps people function in their preferred setting.


How is this different from Careforth’s original 2014 accreditation?

Careforth’s original 2014 accreditation was NCQA’s Case Management Accreditation. In 2017, Careforth added the LTSS-specific distinction, and this year’s survey—following renewals in 2020 and 2023—reaffirms Careforth’s full 3-year Case Management for LTSS accreditation, a status held by just 76 organizations nationwide.
