Careforth Celebrates Case Management Week 2024

We’re proud to celebrate Case Management Week, an annual opportunity to recognize the important and impactful work case managers do on a daily basis. At Careforth, these professionals are called care managers. These heroes in healthcare are a source of support, advice, and comfort to caregivers and the loved ones they care for. Read on to hear from Careforth care managers and learn why their work is so vital to the caregiving community!

What Is Case Management and Why Is It Important to Caregivers?

Careforth care managers are a vital part of the care teams that support caregivers and their families. They’re a helping hand, a listening ear, and a source of guidance for the families they work with each day. When they’re not conducting home visits, they’re creating personalized care plans, assessing needs, and connecting caregivers with the resources they need to confidently care for their loved one at home. 

“One of the ways I support families is by visiting them on a monthly basis. Some of them don’t get into the community much, and I serve as a bridge between caregivers, the person they care for, and their available community resources,” said RN Care Management Lead Meghan Goolishian.

Why Case Managers Matter to Caregivers

Careforth care managers are at the heart of personalized care. Each family’s needs are different, and care managers help make sure those needs are met. This can include equipment access, fall prevention strategies, or navigating community resources available to them. Care managers also:

  • Advocate for patients. “With my knowledge of chronic illness and the healthcare system, I coach my caregivers how to be an advocate for the ones they are caring for,” said RN Care Manager Melissa McManus.
  • Improve healthcare outcomes. “I’ve had the honor of working for Careforth for the last 6 years and it’s been an amazing journey. Our team has continued to grow and change in order to provide the best outcomes for the families that we serve,” said Care Coach April Scherer.
  • Enhance efficiency. “I find great satisfaction in being a reliable support system for the families we serve helping them navigate complex healthcare needs with compassion and professionalism,” said Care Manager Luis Disla.
  • Support families. “I love the personal connection I have with the consumers and caregivers I work with. It’s a very rewarding feeling to see them achieve their goals and be at their side each step of the way,” said RN Care Manager Karen Morgan.
  • Build relationships. “I believe it’s crucial to let families know they are not alone on their journey, as I also care for someone dear to me. The best part of my job is celebrating both the little wins and the major milestones we achieve together,” said Care Manager Julia Mooney.

As dedicated care managers, these professionals help caregivers create a safe environment for their loved ones to thrive at home. Careforth’s Structured Family Caregiving (SFC) model, and our care teams that support caregivers, positively impact outcomes for care recipients. Earlier this year, ATI Advisory conducted an outcomes analysis on behalf of Careforth revealing that care recipients enrolled in our SFC service have a 46% lower probability of at least 1 injurious fall in a year and a 21% lower probability of at least 1 emergency department visit in a year.

The work that Careforth care managers do each day makes a significant difference in the safety and confidence of caregivers and their loved ones.
