Careforth celebrates achieving NCQA accreditation for its Caregiver Homes Network again
We are happy to announce that Careforth’s Caregiver Homes Network Structured Family Caregiving service has again received a three-year Accreditation for Case Management with Long Term Services and Supports (LTSS) Distinction from the National Committee for Quality Assurance (NCQA). This marks the third time we have been awarded three-year accreditation, which is the highest accreditation status awarded by NCQA, demonstrating Careforth’s strong performance of NCQA’s rigorous requirements for case management.
NCQA is the gold standard for health care quality assessment. The committee provides a road map for organizations to achieve quality care and the same required standards are evaluated for all organizations seeking to be accredited. This creates a fair and comprehensive comparison of organizations across the country.
Careforth’s Commitment to Supporting Caregivers
“Achieving this is no accident; NCQA’s requirements are Careforth’s standard. Moreover, it is a testament to everyone’s hard work and dedication to providing quality care and supporting caregivers at every turn. In this, it is truly a company-wide initiative and achievement,” said Careforth Clinical Quality & Performance Improvement Manager Kristen Reming. “This accreditation reflects the countless hours our team has spent providing the highest quality of care to the families we serve and further demonstrates our commitment to family caregivers.”
From our Care Team and Coaches working with the families we serve, to Clinical Directors overseeing clinical quality across state partnerships, to our technologies team for developing a digital collaboration app to provide a HIPPA-secure care platform, every single member of our Careforth team has helped us achieve and maintain this re-accreditation.
“Receiving this endorsement is a testament to the dedicated Care Management professionals at Careforth,” said Careforth President and Chief Executive Officer Matt Marek. “It’s truly an honor for an organization like NCQA to recognize our commitment to delivering high quality case management to the families in our Structured Family Caregiving service. We intend to keep using the NCQA standards as our North Star for continuous improvement.” Contact us to learn more about how Careforth supports caregivers.
