Seniorlink presenting at the 2019 Aging in America Conference

Seniorlink’s Laurie Herndon, MSN, GNP-BC, Clinical Program Innovation Director, and Saran Craig, Clinical Program Innovation Manager, will be speaking at the 2019 Aging in America Conference on April 18th in New Orleans, Louisiana. The session, Tech-Enabled Dementia Care Pilot Leverages Coach to Drive Outcomes and Improve Caregiver Engagement will take place during the 90-minute symposium “Innovative Tech-Enabled Approaches for Dementia Care.” The session will discuss Seniorlink’s dementia care pilot, a tech-enabled program that leverages a remote coach to support caregivers of individuals with dementia. Guided by evidence-based pathways, the coach works through Vela, Seniorlink’s care collaboration platform, to engage caregivers in a person-centered manner. This session will detail the approach, the methods of implementation, including lessons learned, and the assessment of outcomes.
