Faces of Careforth: Brittany Nali
Case Management Week is about more than acknowledging those who coordinate care. These professionals build trust, listen closely, and are there when families need someone in their corner. For Careforth Care Manager Brittany Nali, that means getting to know the families she supports, understanding what matters most to them, and providing the guidance, resources, and encouragement they need to navigate caregiving. That above-and-beyond support deserves to be celebrated.
What is your role and how long have you worked at Careforth?
I am a care manager and have been with Careforth since September 2023. In my role, I conduct monthly in-home visits with families enrolled in the Shared Living Program. I provide ongoing support, guidance, and coaching, and I connect caregivers with appropriate resources when needed. I also monitor the overall well-being of both the caregiver and the person they care for, ensuring that they are in a safe, healthy, and supportive environment.
What inspired you to join our team?
I have a strong passion for case management and am committed to supporting individuals and families in a meaningful way. When exploring opportunities, I was drawn to Careforth because of its strong reputation and the positive things I have heard about the organization, particularly its commitment to supporting a healthy work-life balance for its employees. I also feel a strong connection to Careforth’s mission. I value the opportunity to help individuals remain safely and comfortably in their own homes while receiving the care and support they need. I appreciate that Careforth also recognizes and supports family caregivers, helping families remain together while alleviating some of the emotional and financial challenges that can come with caring for a loved one.
How do you support families and caregivers in your role, and why is it important?
One of the most important aspects of my role is being a reliable and compassionate source of support for caregivers. Caregiving can come with many emotional, physical, and practical challenges, and I make it a priority to provide caregivers with a safe and supportive person they can turn to when they need to talk, ask questions, or work through a difficult situation.
I also make sure that families know they can reach out to me whenever they feel they need additional support. I believe that consistent communication helps build trust and allows me to better understand their individual needs. In addition to providing emotional support, I offer coaching, guidance, and resources tailored to each family’s circumstances. I assist them in navigating challenges, identifying appropriate resources, and understanding the expectations and guidelines of the Shared Living Program.
Do you have a personal connection to caregiving that you’re comfortable sharing? If so, what is it?
While I do not have personal experience as a caregiver myself, I have witnessed first-hand the responsibilities and challenges that come with caregiving. My mother was the primary caregiver for my grandmother and eventually welcomed her into our home so that she could provide her with the daily care and support she needed. At the time, my mother was also working, which often created significant challenges as she tried to balance the demands of her job with the needs of my grandmother.
That experience gave me a deeper understanding of the physical, emotional, and financial pressures that family caregivers can face. It also helped me recognize how valuable programs like Shared Living can be in supporting both individuals and their families. At the time, the Shared Living Program was not as widely known, and I often find myself wishing that my family had been aware of this type of support. Having access to a program like Shared Living could have alleviated some of the burden my mother experienced and allowed her to focus more fully on caring for my grandmother without having to constantly balance caregiving responsibilities with the demands of working outside the home.
How does your work contribute to Careforth’s mission and values?
My work helps those receiving care remain safely and comfortably in their homes while their caregivers get the support they need.
A significant part of my responsibility is ensuring that families have the knowledge, resources, and tools necessary to provide the best possible care for their loved ones. By offering consistent support and education, I help caregivers navigate the responsibilities of caregiving while promoting the overall safety, well-being, and quality of life of the person they care for.
What’s something interesting about your job that people may not know?
I have the privilege of working with families from a wide variety of cultural, personal, and social backgrounds. This has been such a rewarding aspect of my work as a care manager. It has provided me with the opportunity to learn from the families I support and gain a deeper appreciation for the many traditions, values, and experiences that shape their lives. Through these relationships, I have had the opportunity to learn about different cultural traditions, holiday celebrations, family practices, and day-to-day customs. I value when families share these experiences with me, as it allows me to better understand what is meaningful and important to them.
How do you maintain a positive work-life balance in this field?
Staying organized allows me to manage my workload efficiently, remain focused on important tasks, and ensure that deadlines are consistently met. Completing my responsibilities, staying organized, and maintaining appropriate boundaries helps me make time for my personal life, family, and interests.
What advice would you give to someone considering a career in the healthcare space?
It is important to approach everyone with an open mind, patience, and a genuine desire to help. Every person has unique needs, experiences, and challenges, and taking the time to listen and understand their perspective can make a meaningful difference in the care and support they receive. Working in healthcare can be incredibly rewarding. Even small acts of encouragement or advocacy can have a lasting impact on an individual and their family. Often, you may not immediately realize the difference you have made in someone’s life, which is why approaching the work with compassion and dedication is so important. For those who genuinely enjoy helping others and building meaningful connections, a career in healthcare can be both personally and professionally fulfilling.
What do you think makes a great case manager?
Being a successful case manager requires a combination of professional skills and personal qualities. In my experience, some of the most important are empathy, compassion, strong communication, advocacy, responsiveness, and the ability to build trust. A great case manager takes the time to understand each family’s individual needs, listens without judgment, and provides support in a respectful and compassionate manner.
What is one fun fact about you that your colleagues may not know about?
I have played competitive softball since the age of 8 and just retired 2 years ago. I now coach my daughter.
